The Shoulder Pain and Disability Index (SPADI) focuses on evaluating the impact of shoulder pain and functional limitations on a patient’s daily activities. It’s designed specifically for individuals with shoulder pathologies, making it a valuable tool across both conservative and operative care pathways.

The SPADI is a validated, widely adopted patient-reported outcome measure (PROM) that quantifies shoulder-related pain and disability. It supports the assessment of treatment efficacy and tracks patient progress over time, making it useful in physical therapy, orthopedic, and surgical settings.

The Shoulder Pain and Disability Index is a focused PROM that evaluates how shoulder pain and mobility issues affect daily tasks, including dressing, lifting, and reaching. It provides insights into the degree of pain and functional limitation, allowing clinicians to track treatment effectiveness and fine-tune care strategies. SPADI is commonly used before and after clinical intervention to assess recovery and monitor long-term progress.

Use Cases:

  • Primary Use: Measures shoulder-related pain and limitations in movement or daily activities.
  • Target Conditions: Rotator cuff tears, adhesive capsulitis (frozen shoulder), osteoarthritis, tendinopathy, post-traumatic stiffness
  • Post-Surgery Assessment: Tracks post-operative recovery, functional return, and pain reduction after arthroscopy, repair, or replacement.
  • Other Applicable Patients: Individuals undergoing conservative care such as physical therapy, injections, or non-surgical orthopedic management.

The SAPDI includes 13 questions, grouped into 2 subscales:

Pain Subscale (5 items)

Assesses the intensity of pain during specific shoulder movements and activities:

  1. At rest
  2. During lifting
  3. Reaching
  4. Touching the back
  5. Carrying weight

 

Disability Subscale (8 items)

Evaluates difficulty in performing everyday tasks:

  1. Washing back
  2. Putting on a shirt
  3. Putting on pants
  4. Using utensils
  5. Lifting 10 lbs above shoulder
  6. Placing object on shelf
  7. Doing work or usual activities
