Medicare Physician Fee Schedule: CMS Update

The Centers for Medicare & Medicaid Services (CMS) has recalculated the Medicare Physician Fee Schedule (MPFS) for the calendar year 2021, and has announced a 3.75% increase in MPFS payments. This update has been made, post the reversed rate cuts as part of the COVID-19 stimulus package in the final rule.
Congress has made this announcement as part of the Consolidated Appropriations Act 2021 that increased the Physician fee schedule payments by 3.75% to advance payments for providers and other healthcare professionals—offering relentless support during these challenging times. CMS has taken several initiatives and developed comprehensive strategies throughout the pandemic times—to enhance support for the hospitals, Physicians and patients. Some of the key initiatives include support around increasing hospital capacity (Hospital Without Walls), Acute Hospital Care at Home Program, Accelerated & Advance Payment (AAP) Program, relaxation of telemedicine regulations, and other sweeping changes on payment policies and regulatory flexibilities.

The COVID-19 Stimulus package valued at a massive $1.4 Trillion has also facilitated payments for Healthcare Common Procedure Coding System (HCPCS). This has also significantly impacted the MPFS for 2021. The American Medical Association (AMA) reported that the code for the E&M visits alone accounted for a staggering $3 billion. The association also added that any delays in integrating these codes would impact the budget-neutrality adjustment for E&M payments, as mentioned in the 2021 MPFS final rule. This rule facilitated a reduction of about 10% for healthcare organizations—that mitigates the impact of any increase in Physician Fee Schedule Payments that will be compensated in other areas.
CMS also plans to increase funds for E&M services for chronic disease management. The agency also announced that the 2021 PFS conversion factor is now at $32.41 with a decrease of $3.68 from the 2020 PFS conversion factor.
